A full -day event featuring 1.5 hours ACHE FACE TO FACE credits, and 4 CEUs for ACCME, ACPE, ANCC, CCMC, NASW and NHA. The non-profit National Readmission Prevention Collaborative hosts its first ever event in Atlanta. ACHE of Georgia members will see two national speakers on care coordination, Dr. Eric Coleman and Dr. Josh Luke, as well as two panels of local hospital experts on care coordination with post acute.
