on January 16 at Maggiano’s in Buckhead for our in-person lunch and learn program, “Integrating Acute to Post Acute Care Settings: Where Do We Go From Here?” The program has been approved for 1.5 hours of ACHE In-Person Education credit.

Acute care hospitals are faced with ever more regulatory requirements and expanding financial penalties all to elevate the standard of care across the healthcare system on a national scale. The regulatory requirements and the corresponding financial penalties are not new phenomena. Most hospitals aggressively develop performance improvement plans that center on the resolution of these deficits for minimization of potential financial exposure.

Driving forces are pushing acute care hospitals to work collaboratively with members of the post-acute care continuum to cohesively manage patient populations through increasingly more aspects of a patient’s post-acute care progression.

This discussion will look at how these post-acute facilities will continue to play an even greater role in supporting our acute care hospitals in the management of patient populations through the continuum of care.

Networking Time
Registration opens at 11:30 am and we will have 30 minutes of networking time before and after the program, with plenty of time for questions and answers during the program. Lunch will be served beginning at 11:30 am.

Our January 16 program will be held at Maggiano’s in Buckhead.

Some of the questions our panelists will cover include:

  1. What does care collaboration along the post-acute care continuum look like?
  2. How do you achieve “buy-in” from partners along the continuum of care?
  3. What are the common challenges in gaining and preserving funding or authorization for post-acute care?
  4. What are “typical” re-admission rates to hospitals by post-acute care providers?
  5. What are some “best practices” with post-acute care providers to enhance collaborative relationships with providers upstream in the acute care hospitals and with providers downstream along the post-acute care continuum?

Attention Graduate Student Members
ACHE of Georgia is offering monthly lunch and learn scholarships to graduate student members. Graduate student members who attend three of our monthly lunch and learns are eligible for a reimbursement of their registration fees. For example, a graduate student who paid the student rate of $20 and attended three meetings in 2025 would receive a reimbursement of $60.

Reimbursements will be on a first-come, first-served basis and no more than three luncheon events per student member. For details, or to submit a request for reimbursement, contact our Scholarship Subcommittee Chair Chet Bhasin.
